CFE top-up insurance in Bhutan: zone 1 rates and no published hospital prices
What the CFE pays in Bhutan in 2026, why no hospital price can be quoted, how to pay and claim from Thimphu, care in another country, the reduced CFE rate, and the checks for a top-up.

For a French expat in Bhutan, the CFE pays at its highest rates but leaves the whole advance with you. Being in zone 1, Bhutan gets a 67% refund of the real cost on any hospital stay you pay and claim, plus a fixed 21 € per consultation, for care given since 1 January 2026. The CFE has no hospital direct billing in Bhutan, and no hospital there publishes its fees for foreign patients. A top-up matters for the 33% of each stay, for treatment you may receive in another country, and for paying the hospital without funding the bill yourself.
What does zone 1 mean for care in Bhutan?
Zone 1 gives the CFE's highest percentages, but only on the lines paid as a share of the real cost. Lines paid as a fixed amount are the same in every zone. The system is explained in what is the CFE.
Paid as a share of the real cost (zone 1) | Paid as a fixed amount (every zone) |
|---|---|
Hospital stay, medicine, surgery, intensive care: 67% | GP or specialist consultation: 21 € |
Chemotherapy: 80%; radiotherapy: 87% | Paediatrician, child aged 0 to 6: 24.50 € |
Imaging and ECG: 55% | Psychiatrist: 36.40 € |
Laboratory tests: 50% | Dentist consultation: 13.80 € |
Medicines: 65% | Vaginal delivery: 3,164.11 € |
Physiotherapy and nursing: 45% | Caesarean: 3,573.64 € |
There is no ceiling on the percentage lines: 67% of a large bill is 67%, and so is the 33% you keep.

What does hospital care cost in Bhutan?
We cannot say. The website of the national referral hospital in Thimphu (JDWNRH) has no fee page, and we found no official notice setting charges for foreign nationals. A news item mentions a special consultation fee introduced in 2010, but it is not a provider price, so it is not used here.
The ECB publishes no reference rate for the ngultrum, so euro figures are impossible anyway. What the CFE pays can still be shown as a share: on an illustrative bill of 10,000 BTN for a hospital stay, the CFE reimburses 6,700 BTN and you keep 3,300 BTN. On a consultation the logic reverses, because the CFE pays 21 € whatever the fee: how much of a ngultrum bill that covers depends on the rate your bank applies. Both are illustrations, not prices. When you ask for a written estimate, ask whether it applies to foreign residents, since no public schedule tells you.

How do you pay and claim from Bhutan?
Bhutan is not among the 33 countries where the CFE runs hospital direct billing, so the CFE never pays a hospital in Bhutan for you. You pay, keep the paid invoice, and claim online from your personal space ("Déposer une feuille de soins étrangère ou TOM"). The amounts and the currency you enter must match the invoice, one claim can carry up to five acts from the same invoice, and the CFE reimburses in euros by bank transfer. For a hospital stay, also ask the hospital to complete the CFE hospital claim form.
Your side of the contract works at a distance too. Contributions are called at the start of each quarter, in euros, and can be paid from abroad by bank card, PayPal, direct debit or transfer from a French or SEPA account. Keep them up to date: the CFE is not bound to reimburse care if contributions were not paid before it took place.

What if you are treated in another country?
Leave Bhutan for treatment and the rate changes with the destination's zone. Treatment in India, also zone 1, is reimbursed at the same 67% for a stay; in Thailand, zone 3, at 42%. Getting there has its own rules: a flight, or a road journey of more than 150 km, needs the CFE's prior agreement, and transport is refunded at set amounts, not at its real cost. It does not organise medical evacuation; insurers sell that as a separate guarantee.

What should a top-up cover in Bhutan?
How a top-up completes the CFE is covered in what a CFE top-up is. With no published prices and no direct billing, these are the points to settle for Bhutan:
Treatment abroad. Check which countries the formula covers and, in each, whether it tops up the CFE on the real cost.
Evacuation. Check whether organised medical evacuation is included, since the CFE only reimburses transport at set amounts with prior agreement.
Hospital payment. Ask whether the insurer can pay or guarantee a hospital bill, in Bhutan or where you would be treated.
The 33% of each stay. Check the annual limit against a serious admission abroad, not only at home.
If one contract instead of two appeals, first-euro insurance or a CFE top-up compares both routes.

Which CFE zone is Bhutan in?
Bhutan is in CFE zone 1, the zone with the highest rates. Since 1 January 2026, the CFE reimburses 67% of the real cost of a hospital stay, 80% of chemotherapy, 55% of imaging, 50% of tests and 65% of medicines. Consultations (21 €) and births (3,164.11 € or 3,573.64 €) are fixed amounts, identical in every zone.
Can the CFE pay a hospital in Thimphu directly?
No. Bhutan is not on the CFE's list of 33 countries with hospital direct billing. You pay the hospital, ask it to complete the CFE hospital form, then claim online with the paid invoice, entering the amounts and currency exactly as invoiced. The CFE reimburses in euros by bank transfer. Avoiding the advance requires a private insurer.
What do hospitals in Bhutan charge foreign residents?
We found no published fee. The national referral hospital in Thimphu (JDWNRH) has no fee page, and we found no official notice of charges for foreign nationals. As an illustration only, on a hospital bill of 10,000 BTN the CFE reimburses 6,700 BTN. Ask for a written estimate before planned care.
If I am treated in India while living in Bhutan, what does the CFE pay?
The CFE applies the zone of the country where you are treated. India is also in zone 1, so a stay there is reimbursed at 67% of the real cost, like one in Bhutan. In Thailand (zone 3) the rate falls to 42%. Air transport and land transport over 150 km need the CFE's prior agreement.
Is there a reduced CFE contribution for a French resident of Bhutan?
There is one reduced rate, the "catégorie aidée": 228 € per quarter for French nationals living outside Europe, registered with the consulate, whose household resources are below 24,030 € a year in 2026, on an individual contract. The request goes through the consulate and is reviewed every three years. Otherwise contributions follow the standard quarterly scale by age.
Do I need a top-up to live in Bhutan with the CFE?
The CFE pays 67% of a stay, so the question is the other 33%, the advance, and care outside Bhutan, where the rate depends on that country's zone. A top-up with treatment-abroad cover, evacuation and a way to pay hospitals addresses all three. Compare that cost with a single first-euro contract.
