CFE top-up insurance in Cambodia: zone 1 and CFE direct billing up to 100%
What the CFE pays in Cambodia in 2026, what its 100% hospital direct billing still leaves you, Royal Phnom Penh Hospital package prices in euros, and the checks for a top-up.

Cambodia is in CFE zone 1, the zone where the CFE pays the most: since 1 January 2026 it reimburses 67% of the real cost of a hospital stay you pay yourself, and Cambodia is one of 24 countries where its hospital direct billing can reach 100% of the invoice for eligible members in the partner network. Consultations remain a fixed 21 €. A top-up in Cambodia is therefore less about the hospital bill inside the network and more about everything around it: outpatient care, private rooms, stays outside the network and members who cannot use direct billing.
How much does the CFE pay in Cambodia in 2026?
The CFE applies the rate of the zone where the care takes place, as a share of the real cost with no reimbursement ceiling, or as a fixed amount. Zone 1 has the highest percentages of the five zones (the system is explained in what is the CFE).
Care in Cambodia | CFE 2026 (zone 1) | How it is paid |
|---|---|---|
Hospital stay in medicine, surgery, intensive care | 67% of the real cost | you pay, then claim online |
Same stay under CFE direct billing | up to 100% of the invoice | the CFE pays the network hospital |
Chemotherapy / radiotherapy | 80% / 87% | pay and claim |
Imaging and ECG / laboratory tests | 55% / 50% | pay and claim |
Medicines / physiotherapy | 65% / 45% | pay and claim |
GP or specialist visit | 21 € | fixed amount per visit |
Birth: vaginal / caesarean | 3,164.11 € / 3,573.64 € | fixed amounts, uncomplicated delivery |

What do Royal Phnom Penh Hospital's packages cost, and what does the CFE pay?
Royal Phnom Penh Hospital publishes its packages as images on its own package pages, "effective until 31 December 2026". We could not find a published consultation fee or nightly room rate there. Euro amounts use the ECB rate of 3 November 2026, 1.1179 USD per euro.
Royal Phnom Penh Hospital package | Price | In euros | What the CFE pays |
|---|---|---|---|
Normal delivery, 2 nights, standard room, physicians' fees and newborn tests included | 2,299 USD | 2,057 € | fixed 3,164.11 €, so at most the bill |
Caesarean, 3 nights, operating room and anaesthetist included | 3,459 USD | 3,094 € | fixed 3,573.64 €, so at most the bill |
Annual health check-up, Silver | 79 USD | 70.67 € | see below |
Annual health check-up, Gold | 159 USD | 142 € | see below |
Executive check-up, over 50 | 1,000 USD | 895 € | see below |
Both delivery packages cost less than the CFE's fixed amount. The CFE describes these amounts as limited to the real expense, so we read them as paying up to the bill, not more: confirm with the CFE before you count on it. What the packages leave out is what you pay: painless delivery and a caesarean timed between 19:00 and 07:00 are printed as extras, and a birth with complications falls outside the fixed amount. A birth under direct billing needs the CFE's prior agreement.
For check-ups, the CFE's prevention line lists a health check of up to 167.69 € every five years. Its table does not say whether a hospital check-up package bought abroad qualifies, so ask the CFE before booking one for that reason.

What does 100% direct billing still leave you to pay?
More than the number suggests. Direct billing ("tiers payant hospitalier") is run with VYV in a partner network, and the CFE writes the rate as "70%, 80% or 100%" depending on the country: in Cambodia it is the top of that range, but always a ceiling. It applies only if you joined individually, not through an employer, on a MondExpat, JeunExpat or RetraitExpat Santé contract, live in Cambodia, have your contributions paid and no waiting period running, and do not hold cover from a CFE partner that already offers its own direct billing abroad.
Even then, four kinds of cost reach you:
comfort items, including a private or semi-private room, television and phone;
companion costs and doctors' fee overruns ("dépassements d'honoraires");
everything outpatient: visits, tests and medicines taken home are not part of direct billing;
any hospital outside the network, where you pay and claim 67%.
The CFE documents we read do not list the network hospitals in Cambodia, so ask the CFE before a planned admission. One more Cambodian detail: the CFE named Cambodia among the countries of its 2026 extension of the Aloïs neurocognitive screening experiment, for members aged 40 to 85.

What should a top-up add in Cambodia?
For how a top-up works in general, read what a CFE top-up is. In Cambodia, the strong zone 1 rates and the 100% direct billing move the checks away from the hospital stay itself:
Outpatient care first. The CFE pays 21 € a visit, 55% of imaging and 50% of tests, with no direct billing. If you or your children see doctors often, this is the gap that adds up.
The private room. Direct billing never pays it. If you want a room of your own, check that the top-up does.
Stays outside the network. 33% of the bill stays with you, with no ceiling. Check that the top-up pays it on the real cost.
Employer contracts. Employees affiliated through their company have no CFE direct billing at all: for them, 33% of every stay is the starting point.
Births with complications. The fixed amounts cover an uncomplicated delivery; check the top-up's maternity limit and waiting period.
If you are young, healthy and rarely see a doctor, compare the CFE plus a top-up with first-euro insurance before deciding.

Which CFE zone is Cambodia in?
Cambodia is in zone 1, the best-reimbursed zone. Since 1 January 2026 the CFE pays 67% of the real cost of a hospital stay you pay and claim, 80% of chemotherapy, 55% of imaging, 50% of laboratory tests, 65% of medicines and a fixed 21 € per consultation. Births are paid with fixed amounts of 3,164.11 € and 3,573.64 €.
Does CFE direct billing really pay 100% in Cambodia?
Up to 100% of the hospital invoice, for eligible members in the CFE's partner network. The rate is a ceiling: private or semi-private rooms, companion costs, fee overruns, outpatient care and home medication are excluded. You must be an individual MondExpat, JeunExpat or RetraitExpat member living in Cambodia, with rights open, and the stay must be in a network hospital.
Does the CFE cover a delivery package at Royal Phnom Penh Hospital?
The normal delivery package costs 2,299 USD (about 2,057 €) and the caesarean package 3,459 USD (about 3,094 €), both below the CFE's fixed amounts of 3,164.11 € and 3,573.64 €. We read those amounts as paying up to the bill; confirm with the CFE. Painless delivery and complications are not in the package price.
Does the CFE pay for a health check-up in Phnom Penh?
The CFE's 2026 table lists a prevention health check of up to 167.69 € every five years. It does not say whether a hospital package abroad qualifies. Royal Phnom Penh Hospital prints check-ups from 79 USD (about 70.67 €, Silver) to 1,000 USD (about 895 €, Executive, over 50). Ask the CFE before booking one for reimbursement.
Can I join the CFE from Cambodia, and when does direct billing start?
You can join at any time while living abroad. Direct billing needs open rights: no waiting period if you apply within three months of becoming eligible, otherwise three months under 45 and six months from 45. Join individually, not only through your employer, if you want access to direct billing. See how to join the CFE.
Do I still need a top-up in Cambodia with 100% direct billing?
Often yes, but for different reasons than elsewhere. Direct billing does not touch consultations, outpatient tests, medicines taken home, private rooms or hospitals outside the network, and it is closed to employer contracts. A top-up pays those gaps within its limits. If you rarely see doctors, a lighter outpatient option may be enough.
