Do you need a CFE top-up in Sri Lanka? Less for the percentage than for the cash flow. Sri Lanka is in CFE zone 1: since 1 January 2026 the CFE reimburses 67% of the real cost of a hospital stay and a fixed 21 € per consultation, but it has no hospital direct billing in Sri Lanka, so you pay the full bill and wait for the CFE's share. A top-up pays the remaining third and, if it has its own arrangements with your hospital, the advance too.

What does the CFE reimburse in Sri Lanka?

The CFE pays a share of the real cost by zone, without a reimbursement ceiling, or a fixed amount for consultations and births; it does not apply French tariffs abroad. For the zones themselves, read what is the CFE.

What do Sri Lankan hospitals publish, and what does the CFE pay on it?

Only health screenings. Asiri's wellness page prints packages for its Matara branch in rupees ("Rs."), with no date; Asiri Central in Colombo, Asiri Surgical and Asiri Galle show no prices on that page. The ECB publishes no reference rate for the Sri Lankan rupee, so we give no euro amounts.

Asiri Matara package

Price as printed

Basic Health Screening

Rs. 10,650

Executive Health Screening

Rs. 20,310

Whole Body Screening, male / female

Rs. 43,000 / Rs. 33,810

A screening is not one CFE line. Blood tests are reimbursed at 50% and an ECG at 55% in zone 1, a doctor's review may be treated as a consultation, and the CFE's prevention line separately lists a health check of up to 167.69 € every five years without saying whether a hospital package abroad qualifies. Ask Asiri for an itemised invoice and ask the CFE which lines apply before booking a package for reimbursement.

For hospital stays, nothing is published. Lanka Hospitals' health check page showed image placeholders only, Durdans lists room types without prices, and Nawaloka's site returned a certificate error. As an illustration only: on a hospital bill of 10,000 LKR, the CFE reimburses 6,700 LKR and 3,300 LKR stays with you.

What does the CFE pay if you have a long-term condition?

The CFE has higher outpatient rates for members with an exonération du ticket modérateur, for example for a long-term condition (ALD), evidenced by an attestation of French Sécurité sociale rights given when you join. In zone 1 these "with exonération" rates are 72% for imaging and ECGs, 65% for laboratory tests, 100% for medicines, 78% for medical supplies and 60% for nursing and physiotherapy. Two limits: the rate on a hospital stay abroad does not change, it stays at 67%, and the table does not state a higher amount for consultations. For a chronic condition in Sri Lanka, that makes outpatient follow-up better covered than the hospital stays themselves.

Why does no CFE direct billing matter in Sri Lanka?

The CFE's list of 33 direct billing countries does not include Sri Lanka. So whatever your contract, you settle the hospital, have the CFE hospital form completed, and claim online with the paid invoice in rupees; the CFE then pays its 67% in euros. On a long admission, that is the whole bill to fund first. The wait afterwards is short if you file quickly: the CFE reports that claims for care abroad filed online were processed in 4.9 days on average in 2025, and you lose the right to claim two years after the first day of the quarter that follows the care. A private insurer is the only way to avoid this advance in Sri Lanka, through its own direct billing or a guarantee of payment.

What should a top-up cover in Sri Lanka?

The general role of a top-up is explained in what a CFE top-up is. For Sri Lanka, these are the questions to ask:

  • Can it pay or guarantee the hospital bill in Colombo, Kandy or Galle, so you do not fund the advance?

  • Does it pay the remaining 33% of a stay on the real cost, and up to what annual limit?

  • How does it reimburse screenings and outpatient tests, which the CFE pays at 50% to 55%?

  • If you have a long-term condition, does it accept it, and does it cover the hospital share the CFE's exonération does not improve?

If you would rather hold one contract, compare with first-euro insurance; for a pre-existing condition, the CFE's lack of a health questionnaire is worth weighing.

Which CFE zone is Sri Lanka in?

Sri Lanka is in CFE zone 1. For care since 1 January 2026, the CFE reimburses 67% of the real cost of a hospital stay, 55% of imaging and ECGs, 50% of tests, 65% of medicines and a fixed 21 € per consultation. Deliveries are fixed amounts: 3,164.11 € vaginal and 3,573.64 € caesarean, without complications.

Does the CFE pay hospitals directly in Sri Lanka?

No. Sri Lanka is not one of the 33 countries where the CFE offers hospital direct billing. Every member pays the hospital, then claims online with the paid invoice in rupees, and the CFE reimburses its 67% in euros by bank transfer. Only a private insurer with its own direct billing or a payment guarantee can avoid the advance.

Does the CFE reimburse a health screening at Asiri?

Partly, and it depends on how it is billed. Asiri Matara prints screenings from Rs. 10,650 (Basic) to Rs. 43,000 (Whole Body, male). The CFE pays tests at 50% and an ECG at 55% in zone 1, and lists a prevention health check of up to 167.69 € every five years. Ask for an itemised invoice and check with the CFE first.

I have a long-term condition: what does the CFE pay in Sri Lanka?

With an exonération du ticket modérateur, proven by an attestation of French Sécurité sociale rights at joining, zone 1 outpatient rates rise: 100% for medicines, 72% for imaging, 65% for tests. A hospital stay abroad stays at 67% either way. The CFE asks no health questionnaire, so a pre-existing condition does not prevent you from joining.

Can I join the CFE while living in Colombo?

Yes, at any time and at any age. If you apply within three months of becoming eligible, your rights start at once; otherwise reimbursements begin after three months under 45 and six months from 45. If you are relying on exonération rates, send the attestation of French Sécurité sociale rights with your application. What should a CFE top-up add in Sri Lanka?

A way to pay the hospital without advancing the bill, since the CFE has no direct billing there, plus the 33% left on every stay and the gap on screenings, tests and consultations. If you have a chronic condition, check how the top-up treats it, because private insurers can apply exclusions the CFE does not.
