For most hospital care in the Philippines, the CFE alone leaves a large share with you. The Philippines are in CFE zone 2: since 1 January 2026 the CFE pays 50% of the real cost of a hospital stay if you pay and claim, up to 80% if you are eligible for its hospital direct billing in the partner network, 40% of imaging and 21 € per consultation. A top-up matters most for three bills the direct billing never touches: outpatient scans, doctor visits and private or semi-private rooms.

What does the CFE pay in the Philippines, and where can direct billing apply?

The CFE reimburses care abroad by zone, as a percentage of the real cost or as a fixed amount, never on French tariffs (see what is the CFE). The Philippines are one of seven countries where its direct billing pays 80%, but that rate only reaches part of what you spend in a year.

What does a Cebu hospital charge, and how much does the CFE pay back?

Chong Hua Hospital in Cebu City publishes its room rates (undated, "subject to change without prior notice") and an imaging price list dated 26 August 2026. Euro amounts use the ECB reference rate of 3 November 2026, 70.943 PHP per euro.

Room lines only illustrate the rule: a real stay adds doctors, tests and medicines, and the 50% applies to the whole bill. In money terms the gap in Cebu is small per day, but it is a share with no ceiling, and it repeats on every scan you have outside a stay. Chong Hua's Mandaue site prints its regular private room at 3,300 PHP a day, so prices also vary between branches of the same hospital: check the rate of the building you are admitted to.

Who can use CFE direct billing in the Philippines?

Direct billing ("tiers payant hospitalier") means the CFE pays its share straight to a hospital of its partner network, run with VYV, so you do not advance it. It is reserved for members who joined individually rather than through an employer, hold a MondExpat, JeunExpat or RetraitExpat Santé contract, live in the Philippines and have their rights open, which means contributions paid and no waiting period running. One more condition catches many people: you lose it if your complementary cover comes from a CFE partner that already provides direct billing with its own network abroad. The older "contrats ex" have not had direct billing since 1 July 2025.

When you do qualify, 80% is a maximum. Comfort costs of a private or semi-private room, companion costs, doctors' fee overruns, outpatient care and home medication stay with you, and a birth needs the CFE's prior agreement. We found no public list of Philippine hospitals in the network: ask the CFE before a planned admission.

Why are doctors' fees the hidden line in the Philippines?

None of the hospital pages checked for this guide publishes doctors' professional fees. Chong Hua prints room and imaging prices only, and St. Luke's Medical Center shows no amounts at all, offering a cost estimate on request. For you, this means the room rate is never the bill. During a stay you pay and claim, the doctors' fees are part of the real cost the CFE reimburses at 50%. Under direct billing, any fee overrun is excluded and comes back to you. Ask the hospital for a written estimate that includes the professional fees before a planned admission.

What should a CFE top-up cover in the Philippines?

The general case for a CFE top-up is explained elsewhere. For the Philippines, check these points against the numbers above:

  • It pays the 60% left on outpatient imaging: 112.02 € on one brain MRI in Cebu.

  • It covers private and semi-private room costs, which CFE direct billing excludes, if you want a room of your own.

  • It pays the remaining 50% of a stay on the real cost, doctors' fees included, when you are outside the network or not eligible.

  • It settles the remaining 20% with the hospital under direct billing, and says whether fee overruns are covered.

  • It does not come from a CFE partner whose own direct billing would cancel yours, unless its network is better for your city.

For some profiles, first-euro insurance costs less than the CFE plus a top-up: compare both.

Which CFE zone are the Philippines in?

The Philippines are in zone 2. Since 1 January 2026 the CFE pays 50% of the real cost of a hospital stay when you pay and claim, 40% of X-rays, scans and MRIs, 50% of laboratory tests, 65% of medicines and 21 € per GP or specialist visit. Births are paid with the fixed amounts of 3,164.11 € (vaginal) and 3,573.64 € (caesarean).

Is there CFE direct billing in Philippine hospitals?

Yes, at 80% of the invoice at most, in the CFE's partner network run with VYV. You must be an individual MondExpat, JeunExpat or RetraitExpat member living in the Philippines, with rights open. Private and semi-private room costs, fee overruns, outpatient care and home medication are excluded. We found no public list of the hospitals concerned: ask the CFE.

How much does the CFE pay for an MRI in Cebu?

40% of the real cost, because outpatient imaging in zone 2 is reimbursed at that rate. Chong Hua Hospital lists a brain MRI at 13,246 PHP, about 186.71 € at the ECB rate of 3 November 2026: the CFE pays about 74.69 € and 112.02 € stays with you. Direct billing does not apply to outpatient scans.

Does the CFE pay doctors' professional fees in the Philippines?

In a hospital stay you pay and claim, yes: doctors' fees are part of the real cost, reimbursed at 50%. Under direct billing, fee overruns are excluded. Outside hospital, a visit is reimbursed at a fixed 21 €. The hospitals we checked do not publish doctors' fees, so ask for a written estimate before a planned stay.

Can I join the CFE once I live in Manila or Cebu?

Yes. Joining is open at any time from abroad, with no health questionnaire and no age limit. Apply within three months of becoming eligible and you have no waiting period; later, rights open after three months under 45 or six months from 45, and direct billing waits too. Steps are in our guide to <a href="https://alea.care/blog/how-do-i-join-the-cfe">joining the CFE</a>.

What does a CFE top-up add in the Philippines?

Mostly what CFE direct billing cannot reach: the 60% left on outpatient scans, consultations above 21 €, private or semi-private room costs and doctors' fee overruns. It also pays the rest of a stay, 50% outside the network or 20% inside it, within its own limits. Compare its network and outpatient limits with your usual hospital.
