Care in India falls under CFE zone 1: since 1 January 2026, two thirds (67%) of a hospital's real bill comes back to you, and each consultation earns a flat 21 €. No Indian hospital is covered by CFE direct billing: you pay, often with a deposit at admission, and claim afterwards. Two features of Indian private hospital pricing shape what is left to you: tariffs that add a surcharge for foreign nationals, and births billed line by line rather than as a package. A top-up in India pays the remaining third of a stay and can also spare you the advance.

What does the CFE pay in India in 2026?

French tariffs play no part in India: the CFE refunds a percentage of the real bill, uncapped, or a flat sum, depending on the zone (what is the CFE covers the zone system). For zone 1, the shares are:

What does a Mumbai hospital charge a foreign patient?

Lilavati Hospital and Research Centre, in Bandra, publishes its tariffs as scanned PDFs headed "Applicable to Self Paying Patients Only" and "effective from 10th June 2025". Two printed rules matter for a French expat: "Surcharge @ 30% will be applicable to Foreign Nationals, NRIs, PIO and OCI Patients", and the minimum class allowed for these patients is twin sharing. The table applies the surcharge to three daily lines, which is our arithmetic; the hospital does not print surcharged amounts. Euros at the ECB rate of 3 November 2026, 105.9155 INR per euro.

These lines only show the mechanism: a stay adds procedures, medicines and investigations, which Lilavati bills "at actuals". Because the CFE pays a share of what you are actually billed, a surcharged invoice raises both the CFE's 67% and your 33%. The tariff is "subject to revision without prior notice", and the hospital page carries an older note dated July 2022, so check the current sheet before admission.

How are births billed in India, and what does the CFE pay?

Lilavati prints no all-in delivery package. Its maternity tariff lists separate lines, which is why we give no total. For a normal delivery in twin class: labour room 34,850 INR (329 €), gynaecologist's standard recommended charges 66,900 INR (632 €), associate gynaecologist 20,100 INR (190 €), paediatrician 5,900 INR (55.70 €) and anaesthetist 18,100 INR (171 €), plus bed charges, medicines and investigations at actuals. The gynaecologist's fee is "as agreed between the doctor and patient". Registration costs 10,000 INR (94.41 €), non-refundable, and the hospital asks an advance deposit of 220,500 INR (2,082 €) for a twin-class normal delivery. A deposit is not the final bill.

The CFE pays a fixed 3,164.11 € for a vaginal delivery and 3,573.64 € for a caesarean, for an uncomplicated birth, practitioners' fees included. Whether that covers your bill depends on the lines you add up and the surcharge; a delivery with complications falls outside the fixed amount. One condition applies to late members: if your CFE membership began more than three months after you left France, the birth is covered only when conception followed that date.

Why does the lack of CFE direct billing matter in India?

India is outside the 33 countries covered by CFE hospital direct billing. You therefore pay every stay yourself, then upload the paid rupee invoice online, and the CFE sends its share in euros by bank transfer. With hospitals that print advance deposits, the cash you need at admission can be well above your final 33%. Taking that advance off your hands is a job for a private insurer, either by direct billing of its own or by guaranteeing payment to the hospital.

What should a top-up cover in India?

In India, these are the points to check (for the general role, see what a CFE top-up is):

  • Payment at admission. Does the top-up settle or guarantee the bill with your hospital, so you do not fund the deposit?

  • The 33% on the real bill. Including any foreign-national surcharge: check that the top-up pays on the amount invoiced, not on a schedule.

  • Room class. Foreign nationals cannot take Lilavati's cheapest classes, so check the room limit of the top-up against twin sharing or better.

  • Maternity. A Lilavati-style bill built line by line can exceed the CFE's fixed amount, so compare it with the top-up's maternity limit, and note its waiting period.

  • Outpatient care. With 21 € per visit and 50% to 55% of tests from the CFE, frequent doctor visits in India make the top-up's outpatient limit worth reading.

For a single contract instead of two, compare with first-euro insurance.

Which CFE zone is India in?

India is in zone 1. For care given since 1 January 2026, the CFE reimburses 67% of the real cost of a hospital stay, 55% of imaging and ECGs, 50% of laboratory tests, 65% of medicines and a fixed 21 € per GP or specialist consultation. Births are fixed amounts: 3,164.11 € for a vaginal delivery and 3,573.64 € for a caesarean.

Does the CFE offer direct billing in Indian hospitals?

No. India is not on the CFE's list of 33 countries with hospital direct billing. You pay the hospital, often including an advance deposit at admission, then claim online with the paid invoice, and the CFE reimburses in euros. Only a private insurer with its own direct billing or a payment guarantee can avoid the advance.

Does a 30% foreign-national surcharge change what the CFE pays in India?

Lilavati Hospital's self-pay tariff prints a 30% surcharge for foreign nationals, NRIs, PIO and OCI patients. The CFE reimburses a share of the real cost billed, so on a surcharged invoice it pays 67% of the higher amount, and your 33% is higher too. A twin bed at 4,975 INR becomes about 61.06 €, of which the CFE pays about 40.91 €.

What does the CFE pay for a birth at Lilavati Hospital in Mumbai?

A fixed 3,164.11 € for a vaginal delivery or 3,573.64 € for a caesarean without complications. Lilavati bills births line by line: for a twin-class normal delivery, the gynaecologist's standard charges alone are 66,900 INR (about 632 €) and the advance deposit is 220,500 INR (about 2,082 €). Add the lines, and the surcharge, before comparing.

Can I join the CFE while living in Mumbai, Delhi or Bangalore?

Yes. French nationals living abroad can join at any time, with no health questionnaire. If you apply within three months of becoming eligible, there is no waiting period; later, reimbursements start after three months under 45 or six months from 45. For a planned birth, conception must follow your membership date if you joined late. See <a href="https://alea.care/blog/how-do-i-join-the-cfe">joining the CFE</a>.

What does a CFE top-up add in India?

It pays the 33% the CFE leaves on a stay, surcharge included if it reimburses on the real bill, and the gap on consultations and tests. Many top-ups also pay or guarantee the hospital directly, which avoids funding a deposit, since the CFE has no direct billing in India. Check room limits and maternity cover.
