CFE top-up insurance in Indonesia: 50% of a stay, up to 80% with direct billing
What the CFE pays French expats in Indonesia in 2026, who can use its 80% hospital direct billing, a BIMC Bali surgery package worked through in euros, and the checks for a top-up.

A French expat paying a Bali or Jakarta hospital out of pocket gets back half of the real cost of the stay from the CFE, because Indonesia is in zone 2; that applies to care given since 1 January 2026, and a consultation earns a flat 21 €. Yet Indonesia is also on the CFE's short list of seven direct billing countries, where eligible members treated in its partner network have 80% of the invoice paid to the hospital. On a major operation, the difference between those two rates is thousands of euros, which is where a top-up for a French expat in Indonesia starts.
Two ways the CFE pays a hospital bill in Indonesia
Zone 2 rates apply to the real Indonesian bill, not to French tariffs, and the hospital percentage has no reimbursement ceiling (what is the CFE explains the zones). The two payment routes are shown here on the one surgery price we found published in Indonesia: BIMC Hospital Kuta's total hip replacement package, 180,000,000 IDR, about 8,878 € at the ECB rate of 3 November 2026 (20,275.02 IDR per euro).
How the stay is paid | CFE rate | CFE share on 180,000,000 IDR | Left to you |
|---|---|---|---|
You pay BIMC, then claim from the CFE | 50% of the real cost | about 4,439 € | about 4,439 € |
CFE direct billing, if you are eligible and the hospital is in the network | at most 80% of the invoice | up to about 7,102 € | at least about 1,776 €, plus excluded items |
The package includes three nights in hospital, consumables and medication, three physiotherapy sessions, one physiatrist consultation, take-home medication, a premium implant and hotel transfers. The page does not say whether surgeon and anaesthetist fees are included, and BIMC calls its packages "indicative", with the price confirmed at booking. We do not know whether BIMC is in the CFE's network. Treat the table as a worked example, not a quote.
Other CFE rates in zone 2: 40% of X-rays, scans, MRIs and ECGs, 50% of laboratory tests, 65% of medicines, 60% of chemotherapy and 69% of radiotherapy. A paediatrician seeing a child aged 0 to 6 is reimbursed 24.50 €, and births are fixed amounts: 3,164.11 € for a vaginal delivery, 3,573.64 € for a caesarean without complications.

What else do hospitals in Indonesia publish?
Very little. BIMC publishes packages for its Bali sites, but no consultation fee, nightly room rate or delivery package. In Jakarta, Siloam, RS Pondok Indah, Mayapada and Premier Bintaro publish no room or consultation fee that we could find. The BIMC prices that are published are mostly check-ups:
BIMC Bali package | Price | In euros |
|---|---|---|
Medical check-up, Comprehensive (Kuta): physical exam, ECG, chest X-ray, blood and urine tests | 1,700,000 IDR | 83.85 € |
Medical check-up, Jade (Nusa Dua) | 2,200,000 IDR | 109 € |
Doctor on call, South Bali, from | 1,485,000 IDR | 73.24 € |
A check-up package mixes items the CFE pays at different rates: an ECG or chest X-ray at 40%, blood tests at 50%. One package price does not show that split, so ask the hospital for an itemised invoice if you plan to claim. For a doctor's visit, the CFE pays a fixed 21 € whatever the fee.

Who can use CFE direct billing in Indonesia?
Direct billing ("tiers payant hospitalier") lets the CFE pay its share to the hospital, through a partner network run with VYV, instead of you advancing the money. Each condition below must be met:
Condition | What it means in practice |
|---|---|
Individual membership | if your employer affiliated you to the CFE, you have no direct billing |
MondExpat, JeunExpat or RetraitExpat Santé | the older "contrats ex" lost direct billing on 1 July 2025 |
Resident in Indonesia | the service is tied to the country you live in |
Rights open | contributions paid and no waiting period running |
No partner complementary with its own direct billing | a top-up from a CFE partner that already offers direct billing abroad replaces the CFE's |
Even at a network hospital the CFE pays no more than 80%, and some costs stay outside: the comfort extra for a private or semi-private room, a companion's costs, doctors' fee overruns, outpatient care, home medication and births at home. A birth handled through direct billing requires the CFE's prior agreement. We found no list of Indonesian network hospitals in the CFE's documents, so put the question to the CFE before booking a planned stay.

What should a top-up cover in Indonesia?
The role of a top-up is set out in what a CFE top-up is. For Indonesia, the worked example gives the checks:
The 50% gap on planned surgery. About 4,439 € on one hip replacement if you pay and claim: check that the top-up pays the remainder on the real cost, and its annual limit.
The 20% and the exclusions under direct billing. Ask whether the top-up pays the hospital directly and covers fee overruns and a private room.
Its own network. If the CFE network does not include your hospital in Bali, Jakarta or elsewhere, a top-up with direct billing at that hospital avoids the cash advance.
Outpatient care. Each consultation returns 21 € and tests 40% to 50%; if you see doctors often in Indonesia, the top-up's outpatient limit matters.
Comparing with first-euro insurance is worth it if your employer pays neither.

Which CFE zone is Indonesia in?
Indonesia is in CFE zone 2. For care since 1 January 2026, the CFE pays 50% of the real cost of a hospital stay you pay and claim, 40% of imaging and ECGs, 50% of laboratory tests, 65% of medicines and a fixed 21 € per consultation. With its hospital direct billing, eligible members can get up to 80% of a stay paid to the hospital.
How does CFE direct billing work for expats in Indonesia?
The CFE pays up to 80% of the invoice directly to a hospital in its partner network, run with VYV. It is open to individual MondExpat, JeunExpat and RetraitExpat members living in Indonesia, with contributions paid and no waiting period. Employer contracts, the older "contrats ex" and members whose top-up already has partner direct billing are excluded.
What would the CFE pay on a hip replacement in Bali?
BIMC Kuta publishes an indicative package at 180,000,000 IDR, about 8,878 € at the ECB rate of 3 November 2026. If you pay and claim, the CFE pays 50%, about 4,439 €. Under direct billing, and only if BIMC is in the network, it pays at most 80%, about 7,102 €, leaving at least 1,776 € plus excluded items.
How do I contact the CFE from Indonesia?
The CFE answers by phone on +33 1 64 14 62 62, Monday to Friday from 9:00 to 17:00 Paris time, and by email at infoexpat@cfe.fr. Claims and most requests go through your online personal space. For help choosing a complementary formula for Bali or Jakarta, Alea can compare contracts from more than 50 insurers.
Can a French expat already in Indonesia still join the CFE?
Yes, at any time and at any age, with no health questionnaire. The timing changes the waiting period: none if you apply within three months of becoming eligible, otherwise three months under 45 and six months from 45, during which you pay but are not reimbursed. Our page on waiting periods explains how to stay covered.
What does a CFE top-up add in Indonesia?
It pays what the CFE leaves on a stay, half of the bill if you pay and claim, or the 20% and excluded items under direct billing, and the gap on consultations and tests. Some top-ups also bring their own direct billing at hospitals outside the CFE network. Check the annual limit, the outpatient limit and the network.
