(2026) Assurances Santé pour Expats au Japon : Tout Savoir

Japan attracts many French expatriates every year. However, its healthcare system, which is cost-effective but limited for expatriates and expensive in the private sector, requires careful preparation. Medical expenses can represent a significant financial risk without appropriate coverage.

The experts from Alea explain everything a French expatriate needs to know to protect themselves effectively in Japan.

Why is it important to take out health insurance in Japan?

High medical costs for non-residents

The healthcare system in Japan is characterized by subsidized public access for residents. Expatriates, generally not being immediately eligible for subsidized public schemes, must turn to the private sector where rates are much higher.

Examples of costs in Japan:

  • Consultation with a general practitioner: 5,000 to 10,000 JPY (approximately 30 to 60 Euros)

  • Hospitalization in a private room: 10,000 to 50,000 JPY per night

  • Common surgical procedure (e.g., appendicitis): 200,000 to 500,000 JPY

Often limited employer coverage Even if your employer offers group insurance, it frequently includes low caps, exclusions (dental, optical, maternity), or zero portability in case of job change. Private complementary or primary insurance is therefore crucial for complete protection.

Peace of mind and access to the best care Good international insurance guarantees direct access to networks of private hospitals and clinics, often with direct billing, eliminating cash advances and complex reimbursement procedures in stressful situations.

Why it is important not to rely solely on your employer's health coverage

Carefully review the general terms and conditions of your group health insurance plan. You may find that it does not provide sufficient coverage, or that it does not allow you to keep your coverage if you leave the company.

Insufficient coverage

Even if your employer offers medical insurance, the coverage may not be sufficient if you fall seriously ill.

Most group health insurance contracts have limits, such as annual coverage caps, exclusions for certain treatments, or restrictions on access to private health facilities. This means that if you need extensive medical treatment or specialized care, you could incur significant costs. In such cases, a top-up plan can offer more comprehensive coverage, ensuring you have access to necessary medical care without financial strain.

Lack of portability

Very few expatriates realize that leaving or losing their job while living abroad can have serious consequences on their health coverage. In the vast majority of cases, employer-provided health insurance is not portable. The few portable plans that do exist come with strict conditions.

Why choose international health insurance over travel insurance?

Although both types of insurance are designed for people spending time abroad, each product serves a different purpose.

Make sure you understand the differences, otherwise your coverage abroad might not be suitable and you could find yourself facing unexpected expenses.

Travel insurance

Travel insurance covers emergency treatments while you are in another country for a short period, to allow you to return home once you are sufficiently recovered. It rarely covers long-term medical treatment. If you require further care, you will need to return to your home country. Travel insurance ceases as soon as you return to your country of residence.

International health insurance

International health insurance is designed for those living or working abroad for an extended period. It covers both emergency and routine care and offers expatriates a choice of doctor and healthcare facility, as well as the flexibility to receive treatment anywhere within their coverage region.

With international health insurance, you can visit private healthcare facilities when you need to. Thus, you avoid long waiting periods in the public sector.

The Caisse des Français de l’Étranger (CFE): a solution for French citizens living in Japan

French nationals residing in Japan have the option to subscribe to the Caisse des Français de l'Étranger (CFE), a social security organization specially designed for French people living abroad. The CFE allows members to benefit from health coverage similar to that of the general scheme in France, while offering the freedom to choose their healthcare providers in Japan.

Why choose the CFE?

  • Continuity of social protection: The CFE allows you to maintain your rights to French social security, thus avoiding coverage gaps if you return to France.

  • No health questionnaire: Pre-existing conditions are covered automatically.

  • No waiting period if you return to live in France.

  • Complementarity with private insurance: since the CFE alone is not enough, it is recommended to have a top-up plan to be well covered in Japan and the rest of the world.

As a CFE partner, Alea supports the French community in choosing and subscribing to the right CFE option, and the best top-up plan, to receive the protection best suited to your needs.

5 things to consider to find the best health insurance plan

Here is what to think about when looking for health insurance in Japan

  1. Benefits

Check that the plan adequately covers common expenses in Japan (specialist fees, medication, private hospitalization). Options like maternity or coverage for preventive care are a plus.

  1. Customer service

When searching for an insurance provider, inquire about their customer service. Do they offer 24/7 assistance? Can you communicate with them via live chat or phone? Do they offer services in French?

This may seem secondary at first, but whether you have a medical emergency or a question about your insurance claims, you need to be able to contact your broker or insurer for help.

  1. Direct billing (Tiers payant)

Does your insurance offer direct billing? With direct billing, the insurer can pay the hospital, clinic, or doctor directly, preventing you from having to pay out of pocket and submit a reimbursement claim afterward.

This is therefore an excellent option for French nationals living abroad who find themselves in a situation requiring medical care.

  1. Claims processing

Regarding reimbursement claims, some insurers have a strict policy to pay you quickly, while others can take several months. An efficient claims process saves you time and reduces stress.

  1. Expertise

The role of an insurance broker is to help you find the best plan for you and/or your family, adapted to your needs and your budget.

A health insurance broker will help you to:

  • Review and compare all relevant health insurance plans, including top-up plans

  • Clearly explain how the market works, what the best options are for you, and professionally answer all your questions

  • Show you how deductibles affect your overall healthcare costs

  • Help you navigate complex situations if you encounter unforeseen problems with your claims

  • Defend your interests whenever you need it
